It would be appreciated if this form could be filled in at the end of each Tournament so your intentions are known to the Club.       We actually need names early from Jan 2010 to enable up to book the correct number of names.    This also enables all Tournament Sheets to be finalized early to save time.  If you do not book with the Tournament Director at least 5 days before the Tournament, then entries will be accepted on the day, but ………………..
